Reporting and incident investigation, international and Polish experience supported by the highest standards of the management of safety culture.

At the last international congress HSE in Poznan 2014 the report "Analysis of accidents in the transport of dangerous goods" was presented. It is stated in the report that approximately 85% of the events were caused by people. Similar figures come from other industries and could be treated as a fundamental basement of the incidents and accidents.


  • Creation of world-class professionals, who relies in his actions on new way of thinking and modern and up to date methods of incidents investigation.

The goal of training

  • Classification of incidents.
  • Real cost of the accident, practice on calculation of the Cost Benefit Analysis vs. Return on Investment.
  • What to choose? Digitization (software) or use standard reporting schemes like: Why Tree, Why Five, Tap Root, Kelvin Top set.
  • "Human element" or Human error, our main mistake, but where?
  • Domino theory of the cause analysis of incidents: basic and root causes.
  • Why the resistance of reporting dangerous situations is exist? Hidden accidents.
  • Corrective Action: where are the priorities and why accidents are coming back?
  • Do we count all or if someone is with us after accident (e.g. contractors, visitors).
  • Practical exercises using Why Tree and Bow Tie Analyze.

The meeting is divided into two days, the first day is an analysis of the most important issues in the proposed thematic area, and the other discussions and practical application of lessons learned.

Incident investigation

Incident investigation is one of the methods of monitoring the results and effectiveness of the HSE management system.

  • Open discussion between the participants and lecturer. Why do we still have a problem with the reporting of accidents and what do we need to change within the HSE management system? Discussion on the subject of Safety Culture and management commitment.

Types of accidents, their classification (based on international HSE standards).

  • What gives us the statistic on accidents, what is the actual difference between them? Do we really expect all of accident to be reported?

Investigation process, requirements on preparation and typical steps.

Case Study


1. The practical application of the topics discussed in the course.
2. Practicing two most commonly used systems: Why Tree and Bow Tie Analysis.
3. Discussion on reporting systems using the formula and examples provided by the lecturer.
4. Source and root cause analysis: examples from life.
5. Case study: Makondo BP in the Gulf of Mexico and the Piper Alpha in the UK.

Methods of conducting the training

  • Workshop, interactive form of classes.
  • Lecture, discussion, practical exercises, discussion of case studies based on real problems and events with the use of practical examples. Classic formula of the lecture skillfully combined with elements of the workshop on particular issues of the subject. Participants are involved to ask questions, discussion and exchange of views.
